Acadia Healthcare Case Manager in Wichita Falls, Texas
Our Case Managers are valuable members of the treatment-based team, requiring strong assessment and treatment skills. This is an opportunity to work with all service lines and programs, inpatient and outpatient.
Master's Degree in Social Work or related field required.
Texas State Board Licensee or Licensee-Intern in a Behavioral Health Field required:
CPR and CPI required or obtain upon hire.
Our Case Managers:
Meet with assigned patients to orient the patient to the case manager role and explain treatment goals and/or expectations.
Provide age-appropriate assessment, care, and intervention and effectively apply established interventions with patients and document results per program requirements.
Initiate contact with relevant family members and outpatient providers, after signed release has been obtained, to develop and implement care plan and coordinate discharge planning.
Document all activities related to assessment, treatment, and discharge planning.
Provide individual and family therapy for assigned patients per program schedule and document per program requirements.
Seek clinical supervision and support when complications arise in therapeutic setting or therapy sessions.
Facilitate specialized interventions when needed.
Assist in compiling materials for use in group therapy and/or educational/informational sessions.
Facilitate activities that are appropriate to age, development, needs and interests of the patients served.
Support program schedule by conducting assigned components and maintaining group therapy/education schedule.
Complete syllabus or lesson plan for didactic groups and document as required.
All group facilitation responsibilities are based on arrangements made by the unit Program Director and/or the Director of Clinical Services.
Attend interdisciplinary team meetings to participate in adequate treatment planning for individual patients.
Serve as primary case manager for the treatment team with the patient, family, case workers, outpatient case managers, and other professionals.
Maintain a positive relationship and information exchanges with patients and families.
Interact professionally and respectfully with patients, family members, external entities, and internal staff members.
